We are Six Physio Limited, we are experts in physiotherapy and have been providing physiotherapy services to our patients since 1996.
Our Head office is Carnac House, Cams Hall Estate, Fareham, Hampshire, England, PO16 8UZ.
We are registered with the Information Commissioner’s Office and our registration number is ZA046336.

How do we collect information from you?
We obtain information about you when you use our website, for example, when you contact us about our services and when you register as a patient or attend a class at any of our Six Physio Clinics.
What type of information is collected from you?
The personal information we collect includes your name, address, email address, IP address, and information regarding what pages are accessed and how you’ve interacted with the site. If you make a payment online your card information is not held by us, it is collected by our third party payment processors, who specialise in the secure online capture and processing of credit/debit card transactions. If you are a patient or attend a class we will need to collect additional information such as your medical history.
How is your information used & the lawful basis for processing.

As a provider of healthcare services and assessments, our clinicians are under a legal requirement to document your treatment. These treatment notes will necessarily include data about your health. Further to the legal grounds for processing above we will process health data on the basis: That it is necessary for medical diagnosis and the provision of health care services.Â
When providing healthcare services such as physiotherapy or other musculoskeletal treatments we have a professional obligation to supervise and audit the quality of the treatment provided by our healthcare professionals, where we process patient health data for these purposes the further legal ground for doing this is ensuring high standards of quality and safety of health care.
How long will your data be kept for?
How long we retain your data for will vary from matter to matter but will be determined in accordance will the following criteria:
- The length of time necessary to complete our contract with you;
- Any time limits for establishing or defending legal claims or responding to complaints/incidents;
- Any period necessary to comply with our legal obligations under EU/UK law; and,
- Any periods for retention that is recommended by regulators or professional bodies.
Where Six provide healthcare services we have legal and regulatory obligations for records to be kept for a minimum period of time. We will typically keep your data processed in connection with these services for a period of 10 years, after which time it will be destroyed, if it is no longer required for the lawful purposes for which it was obtained.
What rights do you have over your data?Â
You have the following rights over your data:
- Request access to your personal information (commonly known as a ‘data subject access request’). This enables you to receive a copy of the personal information we hold about you and to check that we are lawfully processing it.
- Request rectification of the personal information that we hold about you. This enables you to have any incomplete or inaccurate information we hold about you corrected.
- Request erasure of your personal information. This enables you to ask us to delete or remove personal information where there is no good reason for us continuing to process it. You also have the right to ask us to delete or remove your personal information where you have exercised your right to object to processing.
- Object to processing of your personal information where we are relying on a legitimate interest and there is something about your situation which makes you want to object to processing on this ground.
- Request the restriction of processing of your personal information. This enables you to ask us to suspend the processing of personal information about you.
- You can withdraw your consent for processing your data at any time. Where the processing of your personal data relies on the consent lawful basis you can withdraw your consent.

Who will your data be shared with?
We will not sell or rent your information to third parties. We will not share your information with third parties for marketing purposes.Â
Six Physio is part of the Asenti Group and your data may be shared with any members of our Group where they are providing Ascenti with management reporting, finance or other services. The Group consists of Six Physio and Ascenti.Â

Your insurers, where you instruct us or they are the instructing party.
Six Physio has a debt recovery process in place whereby we appoint a third party to act on our behalf: only relevant information will be shared pertaining to your appointment dates and outstanding amount. We will also share your contact details​ so they are able to get in touch with you.
Will you transfer my data outside the UK or EEA?
Six Physio will not typically transfer any of your data outside of the UK or EEA.Â
If we do have occasions to transfer your data outside of the UK or EEA to countries not deemed by the ICO to provide an adequate level of personal information protection, the transfer will be based on safeguards that allow us to conduct the transfer in accordance with the data protection legislation, such as the specific contractual clauses approved by the ICO (or European Commission as relevant) providing adequate protection of personal information, and conducting transfer impact assessments to identify and additional safeguards required.
How can you update your information?
