Good morning Guru, I went for a 20 mile run on Saturday morning but by mile 15 had started pulling up with pain on the outside of my left hip, swiftly followed by the left. I should add that this was preceded by discomfort around my left knee pushing off uphill and to some degree down. The knee feels (for want of a better description) as though I lose all power to it when pushing off. This usually wears off but not this time and whilst I’ve had some discomfort around the hip at times this was by far the most extreme. I’ve got the Brighton marathon in 4 weeks followed by London a week later and then a 100km event in May. If last weekend had been a marathon I would have to had pull up and not finish it. Your advice would be welcome bearing in mind my timescales. Thanks, Lee

Sounds like on ITB/TFL issue, which cutting to the chase is most likely due to poor gluteal and pelvic control. If you’ve got time (and you have) them I’d be doing massive amounts, but little bits, often of glut control work. Things like lunges and squats – anything in a
weightbearing, functional position. You must feel it in your butt and NOWHERE else….no hip, no knee, no nothing. Id also be on my foam roller (ITB). Training to sub pain limit and never through it.

If times runs out, and it may, for rehab strategies them a localised, ultrasound jab may still be an option. If this is so, make sure it’s done by someone who can, rather than someone who can’t (but says they can!)
